A Refuncable Proactive Coaching Investment is billed after your first follow up results (4-8 months) as follows:
$350/year/individual (Not payable by third parties CPT code 89990)
$550/couple billed as $275 per individual (Not payable by third parties CPT code 89989)
This is an investment in your ongoing success. If you think it’s too much, check the cost of stents, surgery, rescue and rehab after a stroke or heart attack.
It is billed by text invoice, payable by credit card or check. “Nonbillable or nonpayable” (which really means not recognized by third party payers as payable) code is 89990 (individual) or 89996 (half of a couple). It is not submitted to third parties. It is payment for noncovered extra benefits. It is your personal investment. We appreciate payment as soon as you can.
If you cannot afford this, please email drbacks@thecurecenter.life to request a discount or exception. No details required. However….
If you don’t want to pay it , you are encouraged to pursue the standard care experience and results.
If anyone you refer successfully starts a CURE Plan, $100 will be credited toward your next annual fee.
Finally, if you suffer a stroke or heart attack during Proactive Medicine care, the last paid fee will be refunded, no questions asked.
